Vein of Galen malformation (VGAM) is a complex vascular malformation, characterized by a persistent median prosencephalic venous drainage from the cerebral hemispheres to the basal ganglia. Our patient is a 40-year-old woman who presented with recurrent thalamic and right basal ganglia infarctions associated with headaches and transient episodes of diplopia. She had a history of repeated left occipital headaches for 20 years. Brain magnetic resonance imaging revealed hyperintense signal changes in the bilateral thalamus and basal ganglia consistent with multiple thalamic infarctions. Her computed tomography angiography study revealed an abnormal vessel originating from the left internal carotid artery, which was confirmed on magnetic resonance angiography. Excision of the malformation was performed, and the patient was discharged without deficits. VGAM is a well-described vascular malformation that causes hypertension and may predispose to thrombotic complications, including stroke. The clinical presentation of VGAM includes developmental delay, seizures, headaches, and intracranial hemorrhage. However, the patient described in this report had no history of developmental delay, seizures, or intracranial hemorrhage. VGAM should be suspected in any patient with thalamic and basal ganglia infarctions.
